Just give two weeks' written notice. Here's what my two weeks' notice looked like when I quit:
<Your name> <Your address line 1> <City, state, zip>
<Date of letter>
<Supervisor's full name here> East Village Grill and Bar 1 Dixie Trail Raleigh, NC 27607
Dear <Supervisor's name here>:
Effective two weeks from the date of this letter, I resign my position as <Position here>.
Sincerely,
<Signature here>
<Typed name here>
