Windows Small Business Server Premium 2003 R2...

I'm thinking User CALs because we have machines at the office that stay logged in 24/7 (8 of them) and we need 5 people to be able to access terminal services and/or webmail from home while those stay logged in.

If it were device CALs I'd need 13 and if it were User CALs I'd need only 8 still, correct?

1 User per machine.

I think the general recommendation is what you are thinking...
If you have more users than machines, buy machine CALs. If you have more machines than users, buy user CALs.
Think long term too.

With all the remote access and stuff, I think user CALs is the way to go.

when you purchase the software, you choose user or device cals right? i haven't ever seen the retail box that says user or device cals so i assumed that's how it is done?

For these first five CALs, you get to choose. At the top of the CAL Microsoft Software License Terms in the retail packaging, you can choose to allocate these CALs to users or devices.
