barrels, boxes, zombies, etc.

Games where part of the challenge is remembering how to get somewhere. The ONLY reason this is hard is because games are not immersive enough. In real life, it would be no problem. This is just lazy game design.

random medpacks laying around that somehow magically heal you, ah well i guess that doesn't really bother me since it's necessary for the gameplay

Female characters in fighting games have virtually no armor or other protective garments on, as if their perfectly-toned mid-drifts and luscious thighs are somehow bullet-proof/sword-proof.

Perform some special move/dodge/action that makes the boss expose their weakspot, then shoot the weak spot and repeat until boss is dead.

Actually I hate how enemy characters can take literally hundreds of bullets to kill in the gameplay, yet in cut scenes main characters and bosses point guns at each other as if they were not immune to bullets. (Metal Gear Solid mainly... still good game)

Also: self destruct sequences (aka backtrack entire level with a couple of blinking lights and sirens to escape explosion)

Zillion. If anyone besides me played and beat it.

^^^That's the quickest way for me to put down a game and never go back. Bonus points if the idiot (with what I call moth to the flame AI) can die from friendly fire and has a penchant for jumping in your line of fire.

This happened in final fantasy 9. There's buildup throughout the game that Kuja is the ultimate bad guy. Then when you beat him in the Crystal World, Necron shows up. There was no mention of Necron anywhere else in the game until he appears and grand crosses the shit outta you.

ff7 had a good buildup for sephiroth. ff8 had mentions of sorceress ultimecia throughout the game starting on disc 3. im pretty sure zemus was mentioned in ff4 shortly before battling him. but there was never a single mention of necron throughout the entire game of ff9, so its different.

Yeah the escort or protect levels are very common, and never appreciated, haha.

Also, under-water levels where you can magically find ways to get air be it "air bubbles" or some magic power bestowed upon you by a suit of armor(yeah I'm looking at you Link).

^^^On a related note, I find it odd how you can see perfectly clear underwater in fps's. Sometimes it's explained away by you having a helmet or goggles or something, but how come shit isn't blurry if you don't have that stuff?

Indestructible objects that shouldn't be indestructible.

To go along with that, unkillable NPCs.

Gran Turismo 4:
CPU cars can nudge your car off-balance with very little effort, yet you'd have to literally T-bone them in the corners to get the same effect.

FF-series:
Girly boys as main characters.
English dubbing.

COD4:
Sometimes I can break the dogs' necks, sometimes I can't. Seems completely random.

I guess more in tune with cliches...

Platformers where a desert level, a snow level, and water level all have to make an appearance.
FPSs where things would splatter on your visor, obscuring your vision.

You find the Sword of One Thousand Truths(tm) near the very end of the game, but by that point its mere existence is useless since you've already killed everything without its assistance.

Cliche: Jumping from a ledge to a 1 foot square pillar with a camera that has ADD and decides to switch positions mid flight, and the save point was a half hour back >.<

In most racing games the computer won't let you get too far ahead (and sometimes too far behind) of your opponents.

And I dunno if any of you guys remember RC Pro Am from NES, but sometimes they had that one car that went like 3 times as fast as you could even if you raced perfectly.
