I don't get how/why she refused medical treatment, i'd think if someone had a letter carved in to their face, particularly with a strange knife, why they would refuse medical treatment.

Unless the statement means she drove herself to a hospital and got treatment that way.

And i'd be ironic I think if she actually didn't have health insurance, and thus that's the reason she refused treatment.
