Years ago I hypothesized that this was because they were not enclosing the entire post thread / post reply operation in a transaction block, more than likely using seperate queries for reading the flood control timestamp and pushing the post / thread data.
If so, it'd be possible for repeated clicks of "post reply" or "post thread" to be processed vaguely simultaneously with all of them receiving a cached flood control timestamp from the backend -- this would only happen during transient load spikes or periods of sustained load (e.g. "when shit is lagging").
In fact, assuming they're using a db server that isn't fatally flawed this is pretty much the only way it could be happening, and in the past I've tested the hypothesis by intentionally multi-clicking when the server appeared to be under heavy load to see if I could induce multiposts and, lo, I was able to.
