its not a mental illnesshe probably had a left hemisphere stroke.he has expressive aphasia and he is perseverating.and it is really sad.he might be able to remember how to sing some songs.and apparently his wernicke's area is still intact..........he seems to understand what's being said to him.

he possibly knows what he wants to say.......but the pathway to getting it out is blocked.not everyone says tono............thats just what happens to be "stuck" in his mind.he could also have a tumor in the broca's area of his brain rather than a stroke.broca's area is responsible for forming and expressing speech.a neurologist would be better able to say if he knows what to say in his head or not.global aphasia is extremely disabling in that you have no idea what anyone is saying to you and you cannot communicate with anyone else.this often happens after a devastating left middle cerebral artery occlusive strokeSOMETIMES patients are able to read words and choose appropriate answers, but that is extremely rare.
